血圧を下げ過ぎると虚血性心疾患の頻度が増加するJカーブに関する報告のほとんどが欧米のものである．虚血性心疾患の多い欧米に比べ，本邦では脳血管障害が多く，血圧の予後に及ぼす影響は異なると予想され，本邦での確実な成績が必要である．これを明らかにするため，血圧と心血管事故発生率との関係を後ろ向きに解析した．対象は陳旧性心筋梗塞患者7,568例，心血管事故は心筋梗塞再発(致死性，非致死性)，心不全死，心臓突然死，脳梗塞(致死性，非致死性)，脳出血(致死性，非致死性)の各症例とした．拡張期血圧で分類した各群の心血管事故発生率は，60mmHg以下で1,294例中52例(4.0%)，61-70mmHgで2,435例中82例(3.4%)，71-80mmHgで2,361例中75例(3.2%)，81-90mmHgでは1.058例中26例(2.5%)と最も低くなり，91mmHg以上で420例中19例(4.5%)と最も高かった．60mmHg以下，91mmHg以上と比較し81-90mmHgで有意に心血管事故発生率が低かった(p<0.05)．脳血管障害の多い本邦でも，欧米の結果と同様に陳旧性心筋梗塞患者における拡張期血圧と心血管事故発生率との間にJカーブ現象が存在することを初めて明らかにした．また，拡張期血圧81-90mmHgが背景因子に左右されない独立した心血管事故予防因子であることを見出した．","subitem_description_type":"Abstract"}]},"item_2_description_36":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"本文データはCiNiiから複製したものである。","subitem_description_type":"Other"}]},"item_2_description_41":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_2_publisher_14":{"attribute_name":"出版者
