activator (tPA) を用いた血栓溶解療法は有効である反面，出血性合併症のリスクが上昇すると報告されている．matrix metalloproteinase-2 (MMP-2) およびMMP-9は血管基底膜を構成するcollagen type IVを分解するプロテアーゼであるが，我々はtPAを用いた血栓溶解療法に伴う出血性合併症にMMP-2，-9が関与しているかを検討した．凝血によるラット脳塞栓症モデルに対して虚血6時間後にtPAを静脈内投与し，その後の症状変化と梗塞領域およびMMP-2，-9の活性の変化をzymographyで観察した．そしてtPAによる出血性合併症に対するMMP阻害剤BB-94の治療効果を検討した．非活性型のpro-MMP-9，活性型のcleaved-MMP-9は虚血後6，12，24時間にかけて経時的な発現上昇をみた．虚血12時間後のpro-MMP-9，cleaved-MMP-9発現は，tPA非投与群に比べtPA投与群で有意に上昇した．MMP-2の増加率は両群間に有意差を認めなかった．tPAとBB-94の併用実験ではラット脳塞栓症モデルの24時間以内の死亡率が28.6%であったのに対し，tPAによる再灌流群では50%と上昇し，tPAとBB-94の併用群では9.1%となった．また出血量も併用群において有意に減少した．ラット脳塞栓症モデルにおいて，tPAの投与がMMP-9の発現を増加させることからtPA投与後の出血性合併症にMMP-9が関与することが示唆された．MMP阻害剤はその出血量と死亡率を有意に改善することが確認された．","subitem_description_language":"ja","subitem_description_type":"Abstract"}]},"item_2_description_36":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"本文データはCiNiiから複製したものである。","subitem_description_type":"Other"}]},"item_2_description_41":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_2_publisher_14":{"attribute_name":"出版者
