activator (tPA) を用いた血栓溶解療法は有効である反面,出血性合併症のリスクが上昇すると報告されている.matrix metalloproteinase-2 (MMP-2) およびMMP-9は血管基底膜を構成するcollagen type IVを分解するプロテアーゼであるが,我々はtPAを用いた血栓溶解療法に伴う出血性合併症にMMP-2,-9が関与しているかを検討した.凝血によるラット脳塞栓症モデルに対して虚血6時間後にtPAを静脈内投与し,その後の症状変化と梗塞領域およびMMP-2,-9の活性の変化をzymographyで観察した.そしてtPAによる出血性合併症に対するMMP阻害剤BB-94の治療効果を検討した.非活性型のpro-MMP-9,活性型のcleaved-MMP-9は虚血後6,12,24時間にかけて経時的な発現上昇をみた.虚血12時間後のpro-MMP-9,cleaved-MMP-9発現は,tPA非投与群に比べtPA投与群で有意に上昇した.MMP-2の増加率は両群間に有意差を認めなかった.tPAとBB-94の併用実験ではラット脳塞栓症モデルの24時間以内の死亡率が28.6%であったのに対し,tPAによる再灌流群では50%と上昇し,tPAとBB-94の併用群では9.1%となった.また出血量も併用群において有意に減少した.ラット脳塞栓症モデルにおいて,tPAの投与がMMP-9の発現を増加させることからtPA投与後の出血性合併症にMMP-9が関与することが示唆された.MMP阻害剤はその出血量と死亡率を有意に改善することが確認された.","subitem_description_language":"ja","subitem_description_type":"Abstract"}]},"item_2_description_36":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"本文データはCiNiiから複製したものである。","subitem_description_type":"Other"}]},"item_2_description_41":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_2_publisher_14":{"attribute_name":"出版者
