{"created":"2023-06-20T16:50:54.141836+00:00","id":23518,"links":{},"metadata":{"_buckets":{"deposit":"38592e52-44eb-4b4f-b420-988184da5a6f"},"_deposit":{"created_by":3,"id":"23518","owners":[3],"pid":{"revision_id":0,"type":"depid","value":"23518"},"status":"published"},"_oai":{"id":"oai:kindai.repo.nii.ac.jp:00023518","sets":["14:923:1477:4830"]},"author_link":["43589","43591","43590"],"item_2_biblio_info_21":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2004-12-25","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"3-4","bibliographicPageEnd":"176","bibliographicPageStart":"165","bibliographicVolumeNumber":"29","bibliographic_titles":[{"bibliographic_title":"近畿大学医学雑誌"},{"bibliographic_title":"Medical Journal of Kinki University","bibliographic_titleLang":"en"}]}]},"item_2_description_33":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"[抄録] 肺静脈由来の異所性興奮が発作性心房細動 (paroxysmal atrial fibrillation : PAF) の発症に関わることが報告されている.さらに最近では肺静脈はPAFの発症にのみ関与するのではなく,PAFを維持させる基質 (substrate) としても重要な役割を果たすことが示唆されている.肺静脈の電気生理学的特性はその構造の多様さから,興奮伝導の異方向性様式に言及したものが主であり,自律神経系への肺静脈の関与はほとんど明らかにされていない.そこで我々はPAFの既往をもつ患者ともたない患者において,交感神経刺激薬であるイソプロテレノール (isoproterenol : ISP) に対する肺静脈の反応性の相違を検討した.PAF患者10例 (PAF群) と非PAF患者10例 (control 群) に対し電気生理学的検査を施行.両上肺静脈 (left superior pulmonary vein : LSPVと right superior pulmonary vein : RSPV) 及び左心房 (left atrium : LA) の有効不応期 (effective refractory period : ERP) 及び肺静脈-左房間の伝導時間を無投薬下とISP投与下にて測定した.無投薬下において,control 群,PAF群共に各部位でERPに有意な短縮を認めなかった.ISP投与下において,肺静脈のERPはPAF群にてLAのERPと比し有意に短縮していたが (LSPV : 120±58,RSPV : 158±32 versus LA : 191±22 ms,それぞれp<0.01,p<0.05),control 群では認めなかった.control 群とPAF群のERPの比較では,無投薬下では両上肺静脈のERPに有意な短縮を認めなかったが,ISP投与下では,PAF群の肺静脈で有意にERPの短縮を認めた (LSPV : 194±27 versus 120±58 ms,p<0.01 ;
