心血管事故(心筋梗塞再発，心臓突然死，心不全死，脳出血，脳梗塞)二次予防に，より有効なβ遮断薬の作用種別を明らかにする目的で1989年から2002年に近畿大学医学部附属病院循環器内科で治療したすべての心筋梗塞患者1,615人(発症時平均年齢60.6±11.0歳，平均観察期間16.3±18.9ケ月)を対象にカルテで後ろ向き調査し，β遮断薬非投与群と投与群で心血管事故発生率を比較して心血管事故二次予防効果を判定した．β遮断薬非投与群の心血管事故発生率714人中50人(7.0%；57.4人/1,000人・年)に対し，投与群は901人中41人(4.6%；31.1人/1,000人・年)と有意(p<0.01)に低かった．β遮断薬作用種別の心血管事故発生率はβ_1選択性(+)698人中31人(4.4%；30.2人/1,000人・年)，β_1選択性(-)92人中6人(6.5%；37.2人/1,000人・年)，MSA (+) 103人中6人(5.8%；49.8人/1,000人・年)，MSA (-) 783人中35人(4.5%；29.7人/1,000人・年)，ISA (+) 37人中2人(5.4%；38.2人/1,000人・年，ISA (-) 849人中39人(4.6%；31.3人/1,000人・年)，脂溶性(+)720人中34人(4.7%；34.0人/1,000人・年)，脂溶性 (-) 143人中7人(4.9%；26.0人/1,000人・年)で，β_1選択性(+)，MSA (-)，ISA (-)，脂溶性(+)の心血管事故発生率が有意に少なかった(p<0.05, p<0.01, p<0.05, p<0.05)．したがって，β_1選択性(+)，MSA (-)， ISA
