| 内容記述 | The comparative studies of total and proximal gastrectomy were performed among the subjects of 49 cases of total gastrectomy (TG group) and 24 cases of proximal gastrectomy (PG group) operated at the First Department of Surgery, Kinki University Hospital in the six years from April 1975 to March 1981. In the reconstructive surgery, among 34 cases of curative resection in the total gastrectomy group 24 cases (70.6%) had interposed jejunal procedure, 7 cases Roux-Y method surgery and 3 cases BII method surgery. Among 21 cases of curative resection in the PG group esophagogastrectomy accounted for an overwhelming majority with 20 cases (95.2%) and only one case had interposed jejunal procedure. With regard to localization of the primary tumor 11 cases (32.4%) had operations on three areas and 14 cases (41.2%) on two areas in the 34 cases of curative resection while most cases of curative resection had operations limited to one or two areas in the PG group. The macroscopic classification of the primary tumor showed that 30 cases (61.2%) of the curative resection in TG group had Borrmann 3-4 type and that many cases of curative resection had Borrmann 1-2 type in the PG group. Lymph node metastasis was high with n(+) 73.5% in the curative resection cases of TG group, and n(-) 52.4% in the cases of PG group. The histological type of the primary tumor was 57.1% for undifferentiated type in the TG group and 75% for differentiated type in the PG group. As for the resection cases combined with other organs, many of both groups had operations combined with splenectomy. | |||||||||||||||||||||||||
