University","bibliographic_titleLang":"en"}]}]},"item_2_description_33":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"[抄録]口腔がんとは，舌や歯肉など口腔内に発生するがんの総称であり，数少ない視診・触診が可能ながんの一つである．このことから，現在一部地域にて実施され始めている口腔がん検診は，内視鏡や画像診断なしで口腔がんのスクリーニングを実施することができるため，他領域のがんの検診と比較してより簡便である．一方，口腔粘膜の異常は，がん以外にも様々な疾患があり，初期の口腔がんは口内炎や歯周炎の所見と非常に類似する場合もあることから，その鑑別は時に，口腔外科を専門とする歯科医師でも困難となる．口腔がんの治療は，根治的なものには外科的切除と化学放射線療法がある．当院では耳鼻咽喉科・放射線科・腫瘍内科とともに頭頚部キャンサーボードでその治療に携わっている．口腔がんに対して外科的切除を行った場合，しばしば口腔機能障害を伴う．そのため，手術前には顎骨の３D モデルや口腔内装置の準備を行って機能障害を最小限に抑えることができるよう手術に臨むことが多い．また，術後のフォローでは，構音障害や摂食嚥下機能障害，咀嚼機能障害，審美障害などQOL に関わる課題に対しても治療を行っている．加えて，周術期に生じる口腔合併症に対して歯科衛生士による口腔機能管理を行い，術後は，義歯やインプラントなどの口腔内装置の作製を歯科技工士とともに行い，これらの口腔機能障害を最小限に抑えることに取り組んでいる．口腔がんの早期発見・早期治療は治療による口腔機能障害を抑えることにつながるため，行政としても非常に重要な課題となっており，当科は地域医療に連携して口腔がんに携わっていくことが必要であると考えている．","subitem_description_type":"Abstract"}]},"item_2_description_41":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_2_identifier_registration":{"attribute_name":"ID登録","attribute_value_mlt":[{"subitem_identifier_reg_text":"10.15100/00022066","subitem_identifier_reg_type":"JaLC"}]},"item_2_publisher_14":{"attribute_name":"出版者
