内容記述 | [Abstract]The present study investigated the role of atrial natriuretic peptide (ANP) in the mechanism of anti-hypertensive effects of 13-blockers in patients with essential hypertension. Propranolol and carteolol were used as g-blockers without and with intrinsic sympathominetic activity (ISA). The drugs were administered for 12 weeks in 17 patients. Cardiac output was measured by the dye dilution method. After administration of propranolol, blood pressure, heart rate and cardiac index were significantly reduced (p<0.05, p<0.001 and p<0.01, respectively), and ANP was significantly increased (p<0.01). A significant negative correlation was observed between percent changes in systolic blood pressure and those in ANP (r= —0.594, p<0.05), whereas no correlation was found between percent changes in systolic blood pressure and those in the cardiac index. The administration of carteolol significantly reduced blood pressure (p<0.05), without affecting heart rate or cardiac index. Carteolol significantly increased ANP, daily urinary output and creatinine clearances (p<0.05, p<0.05 and p<0.05. respectively). These increases were observed only in the responder group in which carteolol reduced systolic blood pressure by 10 mmHg or more. A positive correlation was noted between ANP and daily urinary output (r=0.756, p<0.05). These results indicate thatadministration of propranolol or carteolol stimulate intrinsic ANP secretion regardless of the presence or absence of reduced cardiac function.
