内容記述 During the period from 1976 to 1986, 58 patients with complete transposition of the great arteries underwent various types of correction. Among them, 43 patients survived for over one year after the correction. Of the 43 patients, 35 underwent the correction by Mustard procedure, 7 by Jatene procedure and the other patient by Rastelli procedure. The comparative study between atrial and arterial switch repair was performed in order to evaluate the long term results, clinical results, hemodynamics and cardiac function through long term follow up study after the correction. Three late deaths occurred among the patients who received Mustard procedure. The leading cause of late death was not heart failure, but dysrhythmia. Late morbidity in this group was not as high as those of other reports and major late problems were rhythm disturbance and tricuspid incompetence. One late death was found among the patients who underwent Jatene procedure and the major complication after Jatene procedure was right ventricular outflow obstruction. Mortality and morbidity of Jatene procedure are now continuing to decline as experience is gained and techniques are refined. Late postoperative systemic ventricular function after Mustard procedure was remarkably impaired particularly in the patients with transposition of the great arteries and ventricular septal defect (TGA and VSD). In contrast, patients undergoing Jatene procedure showed normal ventricular function even in the late postoperative period. In conclusion, Jatene procedure should be considered as the choice of operation for patients with TGA and VSD. For patients with TGA and intact ventricular septum, Mustard procedure is still a valid corrective method, although Jatene procedure is preferable.
