The ability to make the diagnosis of localized liver cancer has recently been enhanced through the use of computerized tomography, ultrasonography and the identification of tumor markers such as α-fetoprotein. In Japan, however, over 87% of the patients with hepatocellular carcinoma have also liver cirrhosis and/or liver fibrosis. Since many patients with hepatocellular carcinoma die of liver insufficiency after radical hepatectomy, a more effective procedure is needed. In 15 of the patients we treated nodular type localized liver cancer was local excised with an ultrasonic surgical aspirator. According to the results, portal emboli and capsule invasion were frequent in patients with tumors of 6 cm or more in diameter. Capsule invasion was sometimes found in those with tumors of 3 cm or more in diameter. The prognosis was poorer if capsule invasion occurred. Capsule invasion, daughter nodules and portal emboli were not seen in patients with tumors less than 2 cm in diameter, and these patients had good prognosis. These results suggest that local excision is effective in the treatment of liver cancer associated with liver cirrhosis when the tumors are less than 2 cm in diameter. |
