Besides modified hemoglobin solutions, the other main group of preparations competing in the field of blood substitutes are synthetic perfluorochemical emulsions (PFC). Their ability to transport oxygen has proven to be successful by the virtually complete blood substitution of rats with FC-43 preparation. Following comprehensive and profound experimental trials with another emulsion, Fluosol-DA 20% (FDA), the next breakthrough came with clinical application. FDA was used in the treatment of acute anemia, cases of ischemia and hypoxia including that of the brain, PTCA, in the perfusion of transplantation organs, and others. Many of the adverse reactions caused by FDA 20% were found to result from complement activation provoked by the emulsifier, Pluoronic F-68. The latest generation PFC emulsions are designed to be more safe and efficient. The application of some PFC preparations in diagnostics looks very promising. |
