A 61-year-old man was admitted with epigastralgia, vomiting, and diarrhea which developed half a day after ingestion of three Amanita virosa mushrooms. On admission, the AST value, ALT value, and prothrombin time were 994 IU/l, 730 IU/l, and 24%, respectively. Hepatitis virus markers were not detected. Mild impairment of renal function was also noted. The patient lapsed into hepatic coma and a diagnosis of fulminant hepatitis was made. He died on the 28th hospital day despite intensive treatment. Autopsy revealed submassive hepatic necrosis with the proliferation of connective tissue and infiltration by neutrophils. |
