内容記述 Five patients with obstructive sleep apnea (OSA) were treated with mandibular advancement splint (MAS) which held the mandible anteriorly and increased the oropharyngeal and hypopharyngeal dimension, thereby keeping the upper airway open during sleep. There was a significant decrease of apneaic and hypoxemic episodes during sleep after a few weeks of MAS treatment. Psycho-intellectual function was evaluated by psychological tests including STAI (state-trait anxiety inventory), SDS (self-rating depression scale), CMI (Cornell Medical Index) and Y-G (Yatabe-Guilford test) in these patients. Psychological derangements were not rare among patients with OSA. These psychological derangements appeared to be severer when the patient had longer duration of apneaic episodes or severe hypoxemia during sleep. Improvement in apneaic and hypoxemic episodes during sleep along with apnea indices was noted after a few weeks of MAS treatment. In one case, the pharyngeal area was measured by the acoustic reflection method and a significant increase of minimal pharyngeal area which was much larger than the effect of electrical submental stimulation by MAS treatment was disclosed. MAS was well tolerated and effective in the treatment of OSA and may improve psycho-intellectual function of these patients.
