Coronary angiography after percutaneous transluminal coronary angioplasty (PTCA) often shows a hazy image, but it is not known which anatomical structures cause this haziness. The purpose of this study was to quantify the post-PTCA haziness using a combination of coronary angiography and angioscopy. Coronary angioscopy of the target lesion was performed after PTCA in 35 patients. Haziness on the coronary angiogram was defined as an uneven distribution of contrast medium at the site of PTCA. Two methods were used to evaluate the severity of coronary arterial stenosis : Densitometric diameter stenosis and densitometric area stenosis. Densitometric diameter stenosis (C) was used to evaluate the width of the vessel, while densitometric area stenosis (D) was found to be useful for evaluating the density of the contrast medium during coronary angiography. The ratio of densitometric area stenosis to densitometric diameter stenosis (D/C) proved valuable for estimating the density of contrast medium in relation to the width of the vessel. This D/C ratio was also used to quantify haziness since the D/C ratio increased in the presence of an uneven intraluminal density. We found that cases with a D/C ratio exceeding 1.10 were positive for haziness and those with 1.10 or less negative. The D/C ratio for the group with disruption of atheroma was higher than that for the group without (1.44±0.55 vs 1.01±0.25, p<0.02), which suggested some association of such disruption with haziness. In conclusion, a new quantitative index of haziness on coronary angiogram was shown and is thus valuable for evaluating haziness following PTCA. |
