内容記述 The long-term effect of oxygen on primary pulmonary hypertension (PPH) has not been established yet, although 18 years has elapsed since we first reported a case in which remarkable improvement was noted after long-term oxygen administration. Since then, we have treated 8 patients with PH of unknown etiology, either PPH (5 patients) or PH with liver cirrhosis (PHLC) (3 patients) with oxygen and vasodilators. This favorable effect of oxygen was noted even in patients without appreciable hypoxemia. On the average, PaO2 was 64.8±16.3 and 68.5±10.7 Torr, before and after commencement of long-term administration of oxygen respecively. The average mean pulmonary arterial pressure before treatmment was 61.3±16.1 Torr. All of our patients noted at least temporary improvement in exercise tolerance within two weeks after commencement of oxygen therapy. The degree of dyspnea on exertion decreased from grade 3.0±0.8 at the start of long-term oxygen to 1.8±0.5 at best, in New York Heart Association (NYHA) functional class. Four of the five patients with PPH survived for more than five years although one patient died at eight months after discontinuation of oxygen. Two patients with PHLC died of liver insufficiency or hepatoma after one and four years of oxygen therapy, respectively. Decrease of Ppa of more than 10 Torr was observed in all four patients in whom cardiac catheterization was repeated after long-term oxygen administration. Long-term oxygen administration has been safe and effective especially in terms of physical tolerance and survival in patients with pulmonary hypertension of unknown etiology.
