Two cases of spontaneous omental hematoma are reported. Patient 1 was a 22-year-old man who suddenly developed severe abdominal pain. A diagnosis of omental hematoma was made from findings on abdominal ultrasonography (US), computed tomography (CT), and magnetic resonance imaging (MRI). Angiography did not demonstrate the source of bleeding. Since his vital signs remained stable, he was treated conservatively. Patient 2 was a 20-year-old man with upper abdominal pain. A diagnosis of omental hematoma was made by US and CT. Since his vital signs were unstable, an emergency laparotomy was performed and a hematoma was found in the greater omentum. However, a potential source of the bleeding could not be found. His postoperative course was uneventful. Although surgery is the usual treatment for spontaneous omental hematoma, conservative therapy is also an option when the vital signs remain stable, as in Patient 1. |
