内容記述 A new, highly sensitive determination method for C-reactive protein (CRP) using particle counting immunoassay has facilitated more accurate determination of CRP in various body fluids. The established reference values were as follows : CRP in serum was 15 to 3,063 ng/ml for adults (26 to 3,992 ng/ml for males, 11 to 1,672 ng/ml for females) and 9 to 73 ng/ml for newborns. The CRP concentration in cerebrospinal fluid and urine was less than 10 ng/ml and 2 ng/ml, respectively. The CRP level in cerebrospinal fluid was low in viral meningitis, and increased in bacterial meningitis. In brain and spinal cord tumors, intracranial hemorrhage, increased intracranial pressure and neurological diseases other than tumors, the CRP level in cerebrospinal fluid was significantly increased, whereas that in serum was not changed. The urinary CRP level was significant high in patients with urinary tract infection, renal failure, diabetic nephropathy, kidney transplantation, and high serum CRP levels and in those after various operations. However, it was low, less than the determination sensitivity (2 ng/ml) in chronic nephritis, acute nephritis and nephrotic syndrome. The CRP level in puncture fluid was higher in the exudate than transudate. The CRP concentration in the puncture fluid was higher than that in the cerebrospinal fluid or urine, and rather similar to that in serum. However, there were no diagnostic characteristics of diseases. The determination of low CRP levels in body fluids, especially cerebrospinal fluid and urine, as well as in blood, helps to analyze cerebrospinal diseases as well as renal and urinary tract diseases.
