Basic fibroblast growth factor (bFGF), a growth factor potent in promoting angiogenesis, has been shown to reduce infarct size in experimentally induced acute myocardial infarction. However, the effect of bFGF on regional myocardial blood flow (Qm) in the acutely infarcted myocardium has not been clarified. In 19 open-chest dogs, the left anterior descending coronary artery was occluded and animals were allowed to recover for 4 weeks ; in 8 dogs bFGF 30μg was injected as a bolus into the left circumflex coronary artery 5 minutes after occlusion (bFGF group) and in 11 dogs bFGF was not injected (control group). Injection of non-radioactive colored microspheres was used to measure Qm. In the inner layer of the infarcted myocardium, the % Qm 5 seconds and 4 weeks after occlusion was 16±1% (% of that in non-infarcted myocardium) and 24±4%, respectively in the control group : 13±4% and 16±3%, respectively in the bFGF group showing no difference between the two groups. In the outer layer, the % Qm 5 seconds and 4 weeks after occlusion was 26±2% and 74±7%, respectively in the control group, and 26±6% and 50±8%, respectively in the bFGF group, showing again no difference between the two groups. We found that intracoronary bolus injection of 30μg bFGF did not increase Qm in the infarcted myocardium in dogs. This study suggested that the angiogenic action of bFGF cannot explain the observed reduction in infarct size. |
