内容記述 One hundred and nine eyes of 90 patients affected with age-related macular degeneration were examined by both fluorescein and indocyanine green angiography using a fundus camera and a scanning laser ophthalmoscope. The detection rates of these two techniques for locating choroidal neovascularization responsible for macular degeneration were compared. Indocyanine green angiography resulted in more frequent detection of choroidal neovascularization than fluorescein angiography. On the basis of the findings from indocyanine green angiography, age-related macular degeneration was divided into 4 types : Type A, solitary choroidal neovascularization was visible in both the early and late phases of angiography ; Type B, multiple areas of choroidal neovascularization were seen in the early phase, and these were fused to each other and enlarged in the late phase ; Type C, choroidal neovascularization was visible only in the late phase ; and Type D, choroidal neovascularization was absent in both the early and late phases. Types A to C were subdivided into subfoveal choroidal neovascularization (SFCNV) type and extrafoveal choroidal neovascularization (EFCNV) type. The course of visual loss or recovery and the response to dye laser photocoagulation (carried out for some patients) were analyzed in relation to the type of choroidal neovascularization. The responses to dye laser photocoagulation were most favorable for Type A-EFCNV cases. Visual acuity remained unchanged or aggravated in most of Type A-SFCNV, B, C or D cases. These results suggest that this new classification serves as an indicator of the pathologic features and severity of the choroidal neovascularization and allows the prediction of the change in visual acuity in response to dye laser photocoagulation. This classification was found to be useful in deciding a therapeutic policy for this disease.
