Thigh pain has detracted from the clinical success of total hip replacements done without cement. The purpose of the present study was to determine the effect of surface coating of the stem on the prevalence of thigh pain. We examined 25 patients with 82 months follow-up in a series of two J-stems (Osteonics ; Allen-dale, NJ) with the same design, but different surface coatings : Normalized smooth surface (Normalized J-stem) and arc depositional coating (AD J-stem). The prevalence of thigh pain with the normalized J-stem increased from 43 per cent at one year to 78 per cent at five years. There were no patient complaints of thigh pain with the AD J-stem. At final follow-up, all hips with the AD J-stem were stable with osseous ingrowth. However, 81 percent of the normalized stems were stable with fibrous ingrowth or unstable. These data suggest that the sole factor contributing to the prevalence of thigh pain in the normalized J-stem is the absence of the AD surface coating. |
