We encoutered a 58-year-old female patient who was admitted to hospital in cardio-pulmonary arrest due to a cardiovascular accident after being transported by ambulance from a physician's office. On admission, the patient showed deep coma and loss of pupillary light reflex, suggesting interruptation of brain blood flow. We treated the patient with advanced cardiovascular resuscitation and intensive care in the emergency critical care center. One month after admission, she recovered consciousness and pulmonary cardiac function. After her recovery from coma without sequelae such as dementia or mental disturbance, she described her experiences during the preceding period, included memories of perceptions using all five senses, comprising a so-called Near-Death Experience (NDE). The impact of NDE included a brightened outlook on her life after resuscitation. Recently, many ethical considerations have been discussed with regard to death, including brain death, mercy killing, death with dignity and death at home. This report describes a NDE and reconsiders resuscitation under various kinds of terminal care conditions. |
