内容記述 Osteosarcoma typically occurs in the tubular bones of children. Its occurrence in the head and neck region is considered rather rare. We have experienced a recurrent case of osteosarcoma of the maxilla. In June 2001, a 58-year-old Japanese male was diagnosed with osteosarcoma of the maxilla, and partial maxillectomy was performed at another hospital. Around January 2002, bleeding started from the oral tumor, and gradually exacerbated. In February 2002, osteosarcoma was diagnosed via biopsy. The recurrent tumor extensively filled the oral cavity. The tumor did not notably infiltrate the facial skin, orbita, and brain on CT. CT showed partial calcifications, but not cervical lymph node involvement. Total maxillectomy following Weber-Fergusson's technique for skin incision and tracheotomy was performed on April 24, 2002. The tumor was pathologically considered to have been completely resected, and the patient's ADL Score was extremely limited due to right paralysis after cerebral infarction. Therefore, no additional treatment was given. However, he could be observed for lung metastasis at 12 months, and died at 20 months after the operation. The majority of deaths from osteosarcoma of the jaw bones are caused by failure in primary lesion control. Our patient with lung metastasis is rather rare.
