We reported 3 cases of rickettsiosis in Awaji island. The first case (case 1) was of tsutsugamushi disease in 52-year-old man. The sera from the patient showed elevation of antibody titer to Gilliam strain of Rickettsia tsutsugamushi by the indirect immunofluorescence (IF) method from less than 1 : 10 to 1 : 80. One year later, his serum also showed 1 : 40 to spotted fever group rickettsia (SFGR) by the IF method. The second case (case 2) was of spotted fever rickettsiosis (spotted fever) in a 60-year-old man. By the IF method, his sera showed elevation of antibody titer to SFGR. The third case (case 3) was of suspected spotted fever in a 54-year-old man. Weil-Felix titer (OX2) of the patient's serum reached 1 : 40. Administration of chloramphenicol rapidly improved his symptoms. After them, new cases of these diseases have occurred in Awaji island. Now there are 7 cases of tsutsugamushi disease and 4 cases of spotted fever. Tsutsugamushi cases were distributed in the northern area and spotted fever cases were distributed in the southern area. Although the vectors are quite different, the two diseases are clinically similar. All patients exhibited mild liver dysfunction and one with tsutsugamushi disease showed DIC. Weil-Felix titers (OX2) were slightly elevated in all patients with spotted fever. |
