内容記述 An immunohistochemical study of type IV collagen (CIV) in colo-rectal cancers was performed to relate localization of CIV to invasive and metastasis of the cancer. The materials were obtained from colo-rectal cancers removed from 171 patients and formaline fixed paraffin sections were prepared. An immunohistochemical staining was performed with rabbit antihuman CIV antibody as the primary antibody and by the ABC method. CIV was commonly localized on epithelial basement membrane of normal colo-rectal tissues in all cases. CIV was unevenly stained over the primary lesion of colo-rectal cancer. Therefore, the foci were divided into 3 zones : superficial (mucosal and submucosal layers), intermediate and deep zones (upper and lower half parts of lesion from the muscle layer to the front of invasion). The positive cases were defined as those with positive CIV staining in more than 2 layers. The positive cases were found in 83.3% of primary cancer with liver metastasis with significant difference from the rate of 5.0% in those without liver metastasis (P<0.001). The cases with liver metastasis were characterized by remarked CIV staining at the front of the invasive lesion, which frequently maintained a glandular structure and, in the absence of the structure, formed several cancerous cell nests. CIV staining was found around the nests and those invading veins. In liver metastatic lesions, CIV was almost uniformly stained at the basal side of the cancerous foci in 94.1% of the cases. These findings suggest that. cases with positive CIV frequently develop liver metastasis, and CIV is concomitant with cancer and cannot be a barrier to intramural invasion.
