内容記述 Sixty six lymph nodes in 22 cases of uterine cervical cancer were studied to evaluate the regional lymph nodes morphologically and immunohistochemically. Lymph nodes were systematized into four basic patterns, according to the morphological classification described by Tsaklaklides et al. : lymphocyte predominance ; germinal center predominance ; unstimulated ; and lymphocyte depletion. Lymph node lymphocytes and locally infiltrating lymphocytes were examined immunohistochemically using lymphocyte surface antigens (CD4, CD8, CD57 and CD16). In addition, in 231 cases of uterine cervical cancer was investigated the relationship between the lymph-node morphological patterns and nodal metastases, depth of invasion, histological type of primary lesions in CPL and/or WHO classification, number of locally infiltrating lymphocytes, and prognoses of the patients. Among the four lymph-node morphological patterns, "lymphocyte predominance" nodes were found to have the highest values of CD4/8, CD57, and CD16, while "lymphocyte depletion" nodes had the lowest values. In the cervical region, the ratio of CD4/8 rose proportionally to the number of infiltrating lymphocytes encompassing cancer nests, while it dropped in the regional lymph nodes. Nodes affected by metastases had higher rate of CD8-positive cells than the healthy ones. The nodes in cases without metastases had the highest rate of "lymphocyte predominance", the negative nodes in cases with metastases had an intermediate rate, while the lowest rate in this pattern was registered in metastases-positive nodes. In the cases free from nodal metastases, a direct relationship was found between the four lymph-node morphological patterns and the depth of invasion, CPL and/or WHO classification of primary lesions and the magnitude of lymphocyte infiltration around the tumor In the cases with nodal metastases, patients with "lymphocyte predominance" nodes and "germinal center predominance" nodes had significantly higher five-year survival rate, than those with "lymphocyte depletion" nodes. These findings suggest that morphological and immunohistochemical analysis of the regional lymph nodes may have prognostic significance in patients with invasive cervical cancer.
