内容記述 The mechanism of host defensive responses in primary lesions of uterine cervical cancer was explored by analyzing the invasion and subsets of tumor-infiltrating lymphocytes (TIL) and stroma-infiltrating lymphocytes (SIL) as well as immunohistologically examining the tumor basement membrane. In early carcinomas, TIL and SIL infiltration was weak, while their infiltration tended to increase in advanced cancer although there were inter-individual variances in the degree of their infiltration. Between two invasive cancer (pT1b and pT2b), there was no difference in the degree of TIL infiltration. In both these cancer the incidence of lymph node metastasis decreased as TIL infiltration become more intense. When the degree of TIL infiltration was compared to prognosis in pT2b cases, the prognosis tended to be better in cases showing more intense TIL infiltration. In analysis of TIL subsets, most infiltrating cells were found to be CD2 positive. CD8 positive cells and CD57 positive cells were detected in the stroma close to cancer lesions. CD4 positive cells showed a diffuse distribution in the stroma. Local CD4/CD8 ratio tended to decrease as TIL invasion became more intense. Hardly any CD14 positive cells were found in invasive cancer. Analysis of type IV collagen and laminin disclosed that the continuity of the basement membrane was preserved almost completely in stage 0, stage Ia and stage Ib type C cases, but that in stage Ib type PL and stage II cases, the basement membrane was markedly destroyed and its continuity was lost, accompanied by intense SIL infiltration to the defective basement membrane. These results suggest that the presence of basement membrane is a factor affecting the degree of SIL infiltration. Moreover, laminin was detected in the cytoplasm of part of the cancer cells, which suggests that cancer cells are involved in the formation of laminin. Prognosis tended to worsen as the membrane defect became more intense. In conclusion the preservation of the basement membrane may play a significant role in host defensive in early cancer and its destruction in advanced cancer, leads to TIL and SIL infiltration. Different prognosis of advanced cerviccal cancer is associated with the difference in local cell-mediated immunity.
