We reviwed clinico-pathological findings in 55 cases of Pseudomonas aeruginosa bacteremia encountered at our laboratory in the last 5 years. This bacteria is refered to as a secondary invader and a major causative organism of nosocomial infections. The overall mortality rate in the 55 cases, 55% was very high, and mortality rate was 62% in males and 46% in females. Malignancy was present in more than 50% of the cases ; leukemia was particularly prevalent. The mean values for AST, ALT, ALP, total bilirubin and glucose were elevated, while those for total protein and albumin were decreased. Almost 65% of the patients had Hb of less than 10g/dl. WBC was increased in some cases and markedly decreased in others, the latter being patients with leukemic or malignant lymphoma undergoing curative treatment. The higher the CRP, the higher the mortality rate ; all patients with CRP value greater than 40mg/dl died. Evaluation of laboratory test values disclosed that the majority of the patients were anemic or in a state of malnutrition. The primary source of the bacteremia in these patients was the respiratory, urinary, or gastro-intestinal tract or the skin. The serotypes of the P. aeruginosa isolated from the blood were determined. As the G-group was most common, followed by A- and E-group bacteria, we suspect that nosocomial infection was responsible. We conclude that P. aeruginosa bacteria is very difficult to treat and carries an extreamly poor prognosis ; endotoxin shock may occur, and effective antibiotics are few. |
