The effects of β-blocker treatment on hemodynamics in relation to plasma atrial natriuretic peptide (ANP) levels were studied in 16 out-patients with essential hypertension. Cardiac output was measured by the dye dilution method before and after propranolol treatment for 12 weeks. Administration of propranolol for 12 weeks to untreated subjects resulted in a significant (p<0.01) rise in plasma ANP levels (from 38.5±21.7 to 67.9±46.7 pg/ml, m±SD). Blood pressure and heart rate were significantly decreased (p<0.05, p<0.001). On the other hand, the cardiac index was significantly lowered (from 4.16±1.34 to 2.91±0.77 l/min/m^2, p< 0.001) at chronic administration of propranolol, which suggested a reflection of decreased cardiac function. A significant negative correlation was observed between change of systolic blood pressure and change of ANP (r=-0.534, p<0.05). Also a significant negative correlation was observed between change of cardiac index and change of ANP (r=-0.529, p<0.05). These findings suggest that decreased cardiac function stimulates ANP secretion. We believe that the stimulating effect of propranolol on ANP may partly account for its hypotensive action. |
