We report our experienced in the anesthetic management during cerebral aneurysm clipping of two women at 26 and 33 weeks of gestation, respectively. Anesthesia was maintained with nitrous oxide and low-dose halothane or isoflurane, as well as fentanyl. Respiratory condition was maintained at slight hypocapnea and systolic blood pressure was maintained at 100-120 mmHg. Ritodrine was used to inhibit uterine contractions. Cesarean section was performed on the patient at 33 weeks of gestation 8 hours following cerebral aneurysm clipping. The clinical course in the other patient after operation was uneventful, and three months later she delivered a healthy baby by a spontaneous delivery. There are many complications, such as aspiration pneumonia, hypotensive anesthesia, hyperventilation, and reaction after the use of beta-sympathomimetic drugs, which affect the survival of the mother and infant. The anesthetic management of pregnant women with cerebral aneurysm is discussed and the literature reviewed. |
