Gallstone ileus is a relatively rare complication of cholecystolithiasis. This is a case report of gallstone ileus. A 65-year-old Japanese male was admitted with a one week history of moderately epigastralgia and vomiting. There was no remarkable past history. He underwent various examinations. The stones in the ileum were revealed by a gastro-intestinal contrast study. A subsequent ERCP study demonstrated cholecystduodenic fistula. We finally confirmed the fistula by gastro-intestinal fiberscopy. Laparotomy was carried out under the diagnosis of gallstone ileus. The gallstones in the ileum were removed through enterectomy and cholecystectomy. Then the cholecystduodenal fistula was closed. The postoperative progress was uneventful. We chose to remove the gallstone through both enterectomy and fistulectomy judging from the general conditions of the patient. |
