内容記述 The impairment of glucose tolerance has been well recognised in Graves' disease. In the present study, the insulin receptor status of human peripheral monocytes was studied in patients with Graves' disease and primary hypothyroidism as well as in normal subjects. The insulin receptor on human peripheral monocytes was measured by flow cytometry using antiinsulin receptor antibodies as previously reported. The insulin receptor levels measured in monocytes did not change with age in the healthy subjects. The monocyte insulin receptor was significantly lower in hyperthyroid patients with Graves' disease than in normal subjects, but there was no difference in the monocyte receptor between the euthyroid patients with Graves' disease who were under treatment and the normal subjects. In patients with primary hypothyroidism, the level of monocyte insulin receptors did not differ from that in normal subjects. The observation of a negative correlation between the fasting serum insulin level and insulin receptor expression by monocytes in Graves' disease suggests that the down regulation of insulin receptors was exerted by hyperinsulinemia in hyperthyroid patients. The decrease of monocyte insulin receptors during the hyperthyroid state was normalized when hyperthyroid patients were treated to attain euthyroidism with anti-thyroid drugs. In Graves' disease, there was a negative correlation between monocyte insulin receptors and serum level of triiodothyronine or thyroxine. In addition, in vitro experiments demonstrated that high concentrations of triiodothyronine and thyroxine significantly decreased insulin receptor expression by IM-9 cells, a human lymphoblastoid cell line possessing insulin receptors. It is thus possible to conclude that elevation of the thyroid hormone as well as insulin in serum can affect insulin receptor expression by monocytes in patients with Graves' disease.
