We investigated the relation between postoperative low output syndrome (LOS) and mulitiple organ failure (MOF) in 120 adult patients who underwent open heart surgery between 1986 and 1988. LOS was diagnosed when cardiac output was less than 2.2L/min/m^2, and MOF was diagnosed when failure of two or more of five organs (heart, lung, kidney, liver and brain) was observed. After open heart surgery, LOS was diagnosed in 36 patients (30%), among whom the incidence of MOF (53%) was significantly higher than that (7%) in non-LOS group. The overall incidence of MOF was 21%, and the mortality in patients with MOF was 24%. The mortality rate rose with the number of organ failures. On the other hand, the incidence of LOS complication in the MOF group (72%) was significantly higher than that (19%) in the non-MOF group. The most frequent failed organ in the MOF group was the heart (72%), followed by the lung, kidney and liver, respctively. In all patients with three or more organ failures, LOS was present. These results suggest that the ocurrence of MOF after open heart surgery is closely related to LOS. |
