内容記述 | In 216 patients who had undergone surgical treatment of gastric cancer, we investigated whether histopathological factors, DNA ploidy pattern and DNA index (DI) by flow cytometry (FCM) and the labeling index (LI) of proliferating cell nuclear antigen (PCNA) by immunohistochemical staining can be a risk factor of distant metastasis of gastric cancer. In the histopathological factors, the cell-differentiation and venous invasion were important risk factors of distant metastasis of gastric cancer. Studies of DNA ploidy pattern suggested a relationship between DNA aneuploidy and liver metastasis. Furthermore, studies of DI and PCNA LI revealed the absence of recurrence when DI=1.0 and PCNA LI≦40%, but peritoneal dissemination was observed when DI=1.0 and PCNA LI≧50%, while liver metastasis was observed frequently when DI=1.0 and PCNA≧60%. In the case of 1.1≦DI≦1.6 and PCNA LI≦40%, recurrence was not observed, but a possibility of peritoneal dissemination or liver metastasis was very high when PCNA LI≧60%. In addition, when DI≧1.7, the possibility of liver metastasis was extremely high regardless of the presence or absence of cell-differentiation and venous invasion. Thus, the observation of the cell-differentiation and venous invasion in paraffin-embedded tissue of gastric cancer, determination of DNA ploidy pattern and DI from FCM as well as measurement of PCNA LI using immunohistochemical staining were suggested to be very useful for evaluating distant metastasis and malignant potential. | |||||||||||||
