| 内容記述 | Destruction of the extracellular matrix is one of the most important events in the course of invasion or metastasis of cercinoma. In 95 cases of endometrial carcinoma, Cathepsin-D and MMP-9 which are respectively proteinases of Laminin and type IV collagen were evaluated. Immunohistochemical staining was performed by an ABC method. The relationship between these proteinase and histopathological findings ; histpathologocal extent of primary tumor, pathological grading of cancer cells, muscle invasion, lymph node metastasis, vessel permeation, peritoneal washing cytology, and the percent area of Laminin and type IV collagen were studied. Cases with a higher histopathological extent of the primary tumor showed a high positive ratio of Cathepsis-D, and significant differences were noted between the pT1 and pT3 cases (p<0.05), and between the pT2 and pT3 (p<0.01). Cases with a lower grading showed a tendency of a high positive ratio of Cathepsin-D. Cases with a deeper invasion showed a high positive ratio of Cathepsin-D, demonstrating a significant difference in the cases with invasion of less than 1/3 and those with invasion of more than 2/3 (p<0.05). Cases with lymph node metastasis showed a high positive ratio of Cathepsin-D (p<0.05). Cases with vessel permeation showed a high positive ratio of Cathepsin-D (p<0.05). Positive cases with peritoneal washing cytology showed a high positive ratio of Cathepsin-D (p<0.05). Cases with a lower grading showed a tendency of a high positive ratio of MMP-9. Cases with a deeper invasion showed a high positive ratio of MMP-9, and a significant difference was observed between the cases with invasion of less than 1/3 and those with invasion of more than 2/3 (p<0.05). Cases with lymph node metastasis and cases with vessel permeation showed a tendeny of a high positive ratio of MMP-9. No correlation was seen between the percent area of Laminin and Cathepsin-D, or between the percent area of type IV collagen and MMP-9. The survival rate of Cathepsin-D negative cases was favorable compared with the positive cases, and the a difference was significant. No difference in prognosis was observed between the MMP-9 positive and negative cases. These findings suggested the usefulness of Cathepsin-D and MMP-9 as indicators of the invasion and metastasis of carcinoma in endometrial carcinoma. | |||||||||||||
