[Abstract] We report an interesting clinical course and valuable outcome of colchicine treatment in a case of posttransplantation cytomegalovirus (CMV)-associated hemophagocytic syndrome (HPS). The patient initially consulted a doctor because of anemia. The diagnosis was considered to be HPS associated with myelodysplastic syndrome (MDS). Thereafter, anemia and thrombocytopenia progressed and multiple blood transfusions were performed while he was treated with various immunosuppressive treatments, including allogeneic hematopoetic stem cell transplantation (allo-HSCT). Complete chimerism was achieved, but pancytopenia, assumed to be caused by posttransplantation CMV-associated HPS, progressed upon the occurrence of cytomegaloviremia 55 days after transplantation. Plasmapheresis and steroid pulse therapy were not effective for the HPS. An alkaloid, colchicine, was administered to reduce the phagocytic activity of macrophages. Anemia improved after several days of colchicine administration, and blood transfusions became unnecessary. Peripheral leukocytes and platelets slowly recovered thereafter. This is the first report of an HPS case treated with colchicine, and the treatment was effective. |
