背景：バソプレッシンV1a受容体は血管や心筋に豊富に存在し血管収縮や心筋肥大への関与が示唆されており，心不全治療においてV1a受容体遮断による有効性が期待されるが，これまでの報告では結論は出ていない．そこで今回我々は，ダールラット高血圧性心不全モデルを用いて，V1a受容体拮抗薬OPC21268の長期投与による効果，さらにはV2受容体拮抗薬Tolvaptanとの併用効果に関しても検討を行った．         方法：心肥大が完成する11週齢の時点から，OPC21268投与群，Tolvaptan投与群，OPC21268＋Tovlaptan併用群，偽薬投与群Control群の４群に分け生命予後を比較した．心不全期にかけての血圧，心拍数，尿所見の経時的な変化を評価し，さらに心不全期における心臓エコー所見，腎機能所見，心臓・腎臓組織における遺伝子発現および組織学的所見の差異についても検討を加えた．結果：経過中OPC21268投与による血圧への影響は認めず，また生命予後の有意な改善は認められなかった．しかし心不全期における左室内径短縮率の有意な改善が認められ，左室心筋における遺伝子発現にては，V1aR，ANP，βMHCの発現亢進が抑制されていた．腎臓においてはクレアチニンクリアランスの改善に加えて，腎臓組織障害の進展が抑制され，レニンおよびコラーゲンの遺伝子発現が抑制されていた．また，Tolvaptanとの併用により生命予後の改善，心腎障害軽減効果が認められたが，それはTolvaptan単独投与と同等であった．          結論：高血圧性心不全モデルラットにおいて，V1a受容体拮抗薬OPC21268長期投与により予後改善効果は認めらなかったが，心筋ならびに腎臓に対する保護効果が認められた．またTolvaptanとの併用は，Tolvaptan単独投与に勝る効果は得られなかった．","subitem_description_type":"Abstract"}]},"item_2_description_41":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_2_publisher_14":{"attribute_name":"出版者
