ゲフィチニブは選択的EGFRチロシンキナーゼ阻害剤(EGFR-TKI)であり, 非小細胞肺癌においてEGFR遺伝子変異を有する症例では非常に高い奏効率を示すと報告されている.しかし, EGFR-TKIの耐性化が問題となっており, この耐性機序としてEGFR遺伝子の二次変異(T790M)やc-METの増幅, HGF刺激など, 細胞質内シグナルの賦活化によるものが報告されている.一方, 非小細胞肺癌細胞株において, 間葉型を示す細胞は上皮型のものに比べEGFR-TKI感受性が低いことが報告され, EGFR-TKI感受性と上皮間葉移行(EMT)の関連が示唆されている.本研究では, EGFR遺伝子変異陽性の非小細胞肺癌細胞株でゲフィチニブ感受性であるHCC827と, c-METの遺伝子増幅によりゲフィチニブ耐性となったHCC827GR5, HCC827GR6とHGF産生によりゲフィチニブ耐性となったHCC827HGF1, CC827HGF2を比較し, 細胞形態を検討した.この結果, HCC827とHCC827GR5, HCC827GR6では上皮型の細胞形態を示したのに対し, HCC827HGF1及びHCC827HGF2では間葉型の細胞形態を示した.次に, HCC827HGF1及びHCC827HGF2はHCC827及びHCC827GR5, HCC827GR6と比較し, E-cadherinの低発現とvimentinの高発現を認めた.さらに, EMT PCR arrayでHCC827HGF1はHCC827と比べZEB1, ZEB2のmRNAレベルの高発現を認めた.加えて, がん浸潤アッセイでHCC827HGF1はHCC827及びHCC827GR5と比較し, 遊走能及び浸潤能ともに機能亢進を認めた.この結果から, EGFR-TKIに対しHGF産生によりゲフィチニブ耐性となったHCC827HGF株はEMTを起こしていることが示された.このことから,
