目的：肺癌根治術後において，異常な呼吸機能低下をきたす症例がある．この症例を術前に予測するために，現在使用している術後呼吸機能予測式から算出される予測値と実測値との差を生じる原因を解析し，簡便かつ精度の高い新しい術後予測機能予測式を作成した．   対象：2006年１月から2007年12月までの肺癌にて根治肺葉切除術を施行した125症例．     方法：術後呼吸機能低下に影響を与えると考えられる因子として年齢，性別，喫煙習慣，手術時間，重篤な併存疾患，癒着の有無，術式，Body Mass Index（BMI），出血量，輸血の有無，術前動脈血酸素分圧，術前動脈血二酸化炭素分圧の計12項目を設定し，検討した．各因子について単変量解析χ^2検定，およびロジスティック回帰分析を用いて解析し，有意差検定を行った．肺切除後の呼吸機能予測式である         Fppo＝［1－(b－n)/(42－n)］×F      Ｆ：respiratory function，     ppo：predicted postoperative      ｂ：切除予定亜区域支数     ｎ：腫瘍によって閉塞している亜区域支数     を用いて算出する術前の術後呼吸機能予測値と術後実測値を比較し，差のある症例に関して検討した因子のうち有意差のある因子を係数として上記予測式内に組み込むことで新しい術後呼吸機能予測式を作成した．         結果：単変量解析（χ^2検定），ロジスティック回帰分析より重篤な併存疾患にのみ有意差がみられた．「重篤な併存疾患」を係数αとし，予測式内に代入する新しい術後呼吸機能予測式      Fppo＝［1－｛(b－n)/(42－n)｝×α］×F       を考案した．術前予測値の FEV_<1.0>(FEV_<1.0> : forced expiratory volume in 1second），６カ月目のFEV_<1.0>実測値等を代入して求めるとα＝1.83となり，   Fppo＝［1－｛(b－n)/(42－n)｝×1.83］×F    が得られた．この式を用いて，2008年６月から12月に至る患者のうち，根治肺葉切除施行し，重篤な併存疾患を持つ10症例で検証を行うと，10症例中７症例で近似（平均誤差1.79±0.32％）を得た．   結論：術後呼吸機能の異常な低下をもたらす因子として，重篤な併存疾患が重要であることが示唆された．この結果に基づき重篤な併存疾患を有する症例に対する術後呼吸機能予測式     Fppo＝［1－(b－n)/(42－n)×1.83］×F
