の転移診断に基づく胸部食道癌手術における頸部郭清の個別化戦略において，術中転移診断を迅速病理診断に因ることの妥当性を検討した.対象はCTおよびFDG-PETで頸部・上縦隔LN転移陰性の胸部中・下部(Mt/ Lt)食道癌40例で，術中1～2個の106recLN を迅速病理診断し，転移陰性例は頚部郭清を省略して，その診断精度と根治性を検討した.術中迅速病理診断の結果， 106recLN転移陽性は15例，陰性は25例で，陽性例で6例(40％)，陰性例で1例(4％)の頚部LN転移/再発を認めた.術中検索した106recLNの診断は術後病理と全例一致したが，陰性例で，頚部再発の1例を含む4例にて術中未検索のLNに転移を認めた. 4例の転移巣は全て長径1mm以上で，検索していれば十分診断可能であった.術後病理診断によれば，頚部LN転移/再発は106recLN転移陽性群では19例中7例(36.8％)に対し，陰性群の21例では1例もなかった. 5年生存率は3領域群:66.7％， 2領域群:71.3％で有意差は無く，再発形式も同等であった.以上より，頚部・上縦隔LN転移陰性のMt/Lt食道癌における106recLNの術中転移診断による頚部郭清の個別化は，根治性の劣化も無く妥当であった.
